Unusual chemotherapy-related toxicity Trial AVF2107g was a blinded, randomised, placebo-controlled phase III trial of IFL with or without bevacizumab 5mg/kg every 2 weeks as first-line treatment of metastatic CRC. Adverse events in the phase III trial that may be attributable to bevacizumab therapy are shown in this slide. Neither grade 3/4 bleeding nor grade 3/4 proteinuria were increased with bevacizumab. The most common adverse event was hypertension, which was easily managed using oral antihypertensive medication. Data were not been adjusted for differences in time on therapy between the two arms. Hypertension: incidence This slide shows the incidence of hypertensive events in clinical trials of bevacizumab in patients with CRC. Hypertension is the most common grade 3/4 adverse event seen in trials of bevacizumab plus chemotherapy and is consistent irrespective of the chemotherapy regimen to which bevacizumab is added. Grade 3 hypertension requires introduction of standard oral antihypertensives. Hypertension is easily managed in this way, but bevacizumab should be discontinued if hypertension remains uncontrolled. Proteinuria: incidence Proteinuria is a common event reported in approximately 20–40% of patients with CRC treated with bevacizumab. Grade 1 and 2 proteinuria are asymptomatic and are more common that grade 3 proteinuria (≤1%). In general, proteinuria improves after patients stop taking bevacizumab. Grade 4 proteinuria (nephrotic syndrome) has not been reported but would require discontinuation of bevacizumab. Similar results have been reported across all trials of bevacizumab. Thromboembolic events during first-line therapy (AVF2192) Trial AVF2192g was a blinded, randomised phase II trial of 5-FU/LV with or without bevacizumab 5mg/kg every 2 weeks in patients with previously untreated metastatic CRC who were not optimal candidates for first-line irinotecan. In trial AVF2192g,1 the incidence of any thromboembolic event was not increased in patients receiving 5-FU/LV plus bevacizumab compared with chemotherapy plus placebo (18.0% vs 18.3%).2 Kabbinavar FF, Schulz J, McCleod M, et al. Thromboembolic events during first-line therapy (AVF2107) Trial AVF2107g was a blinded, randomised, placebo-controlled phase III trial of IFL with or without bevacizumab 5mg/kg every 2 weeks as first-line treatment of metastatic CRC. The overall incidence of thromboembolic events was similar across the two arms (IFL plus bevacizumab, 19.4% vs IFL plus placebo, 16.2%; p=0.26).1 The incidence of venous thromboembolic events was similar in both arms (16.6% vs 15.2%); the incidence of arterial thromboembolic events was increased in the IFL plus bevacizumab arm compared with the IFL plus placebo arm (3.3% vs 1.0%, respectively).1 Novotny WF, Holmgren E, Nelson B, et al. A subgroup of patients in the phase III trial AVF2107 was treated with IFL plus placebo (n=155), IFL plus bevacizumab (n=150) and 5-FU/LV plus bevacizumab (n=37) 28–60 days after cancer surgery. Only two (1.3%) patients treated with IFL plus bevacizumab had a wound healing complication. No such complications were reported in patients treated with IFL plus placebo or 5-FU/LV plus bevacizumab. These data suggest that initiation of bevacizumab therapy 28–60 days after surgery does not increase the incidence of wound healing/bleeding complications. Bleeding: incidence *Epistaxis + GI haemorrhage Clinical trials have shown that minor bleeding (in the form of epistaxis) is increased in patients with CRC who are receiving bevacizumab. Bevacizumab-associated epistaxis is generally manageable and usually lasts less than 5 minutes. Grade 3/4 bleeding events are reported in ≤5% of patients receiving bevacizumab 5mg/kg every 2 weeks.
