Weekly February 11, 2005 / 54(05);125 Notice to Readers: Caution Regarding Testing for Lyme Disease CDC and the Food and Drug Administration (FDA) have become aware of commercial laboratories that conduct testing for Lyme disease by using assays whose accuracy and clinical usefulness have not been adequately established. These tests include urine antigen tests, immunofluorescent staining for cell wall-- deficient forms of Borrelia burgdorferi, and lymphocyte transformation tests. In addition, some laboratories perform polymerase chain reaction tests for B. burgdorferi DNA on inappropriate specimens such as blood and urine or interpret Western blots using criteria that have not been validated and published in peer- reviewed scientific literature. Initial testing should use an enzyme immunoassay (EIA) or immunofluorescent assay (IFA); specimens yielding positive or equivocal results should be tested further by using a standardized Western immunoblot assay. Specimens negative by a sensitive EIA or IFA do not need further testing. CDC. 7 Questo articolo ha avuto rilevante importanza in quanto ha dimostrato come i criteri utilizzati in NordAmerica per definire positiva un'immunoblot (westwern blot) anti-Borrelia b.sia IgG che IgM fossero inadeguati per i genotipi più frequentemente causa di M. di Lyme in Europa (B. afzelii e B. garinii). In questo articolo viene suggerito di diminuire il numero di bande necessarie per refertare positivo il campione rispetto ai criteri nordamericani, con conseguente aumento delle positività rilevate.

La prima strip è positiva e possono essere rilevate le seguenti bande: p100, VisE, p41, p39, p41(B.garinii), p41(B. afzelii), p18.

Le bande più frequentemente presenti sono quelle relative a OspC (con possibilità di distinguere reattività anticorpale verso B.garinii, B.burgdorferi sensu stricto, B.afzelii), p18 e p39
