“The use of the ophthalmoscope as aid in the diagnosis of encephalic disease is gaining ground quickly, and attention is now being given also to its indications in disease of the spine…Firstly, then, are the accounts of disturbances of the inner eye secondary to spinal disturbances to be trusted? Of this there is a little doubt: it is tolerably certain that disturbances of the optic disc and its neighbourhood is seen to follow disturbances of the spine with sufficient frequency and uniformity to establish the probability of a causal relation between the two events”. THE LANCET, January 15, 1870.

Con questo articolo scritto nel 1870, Sir T. C Con questo articolo scritto nel 1870, Sir T.C. Albutt (1836-1925) segnò il principio dell’interesse scientifico per la neuromielite ottica. Nel 1870 il Dr. Devic era un ragazzino di appena 12 anni. L’articolo pubblicato dal Sir Albutt era intitolato: “On the Ophthalmoscopic signs of spinal diseases”. Sebbene siano trascorsi 135 anni, le domande poste da Albutt sono ancora oggi attuali: “Do disturbance of the optic nerve and retina commonly follow spinal mischief? If so, then what kind of disturbances are they? What reason or reasons can we assign for their occurrence?……scriveva questo Sir Albutt nel 1870.

Nove anni dopo, nel 1879, un dottore tedesco Willhelm Erb (1840-1921) sottolineava l’esistenza di questa malattia neuro-oftalmologica (“Ueber das zusammenkommen von neuritis optica und myelitis subacuta” Arch Psychiat Nervenkr. 1879; 10: 146-148.

Il Dr. Eugène Devic Il Dr. Devic nasce a Lione il 24 Ottobre 1858, diventerà un pupillo del Dr. Bouveret e con lui conseguirà il titolo di dottore nel 1886 con una tesi intitolata: “Des rechutes de la fièvre typhoide”. Nel 1888 si occupò di malattie neurologiche presso l’ospedale di Antiquaille, mentre la sua carriere universitaria durerà a Lione solo 2 anni, 1892-94 nell’Ist. di Anatomia Patologica.

Il Dr. Eugène Devic Negli anni che precedettero la sua biennale attività universitaria 1888-1891, Devic pubblica su Province Médicale una serie di articoli inerenti malattie reumatiche, corea, pertosse, rosolia, tbc e febbre tifoide, tutti in ambito pediatrico. La sua collaborazione con il Dr. Bouveret sviluppa articoli inerenti malattie grastroenteriche. Nel 1903 scrive una monografia sull’ulcera duodenale.

Il Dr. Eugène Devic Tra il 1892 ed il 1917 Devic si occupa di diadattica e segue per la tesi numerosi studenti. Nel 1894 uno di questi, Fernand Gault, descrive 17 casi di neuromielite ottica acuta. La tesi viene presentata durante la prima sessione del Congresso Francese di Medicina a Lione (1894). Lui stesso scriverà: “Ils représent une des nombreuses modalités cliniques constituées par les localisations multiples d’un meme temps divers points du système nerveux central et périphérique”.

Il Dr. Eugène Devic Erroneamente si crede che Devic si sia occupato prevalentemente di “Neurologia”. Devic si interessò soprattutto di febbre tifoide (la sua tesi di laurea) e nonostante la breve collaborazione con Bouveret circa le malattie gastroenteriche, uno dei degli argomenti da lui più trattato fu la cardiologia. Il suo interesse per la cardiologia culmina nel 1897 scrivendo con il Dr. Tripier un trattato di semeiolgia del cuore e dei vasi. Infine tra il 1902 ed il 1907 dirige un corso di Anatomia patologica sempre nella sua città, Lione.

Il Dr. Eugène Devic Ma Devic non fu solo un medico. Dopo il 1894 si dedicò attivamente alla vita politica della città di Lione. Nel periodo 1896-1898 fu eletto nel consiglio cittadino di Lione ed esercitò una qualche funzione politica nel municipio. Devic muore nel 1930. Il 23 Febbraio 1930 scrive sulla rivista “Lyon Medicale” un suo pupillo:………..

“Those who knew Dr. Devic only through his studies did not really know him. He was mostly himself while teaching the basic elements of the service he provided, first of all at Red Cross Hospital and then at the Hospital of Lyon, where he stayed until after World War I. There he educated an elite, an elite which has now dispersed but which despite all still bears the imprint left by its master. He showed how to examine a patient, the patience and the method needed, the modesty, honesty and precision called for when making diagnosis. He was also highly conscentious, the greatest quality any worker can have and the worst vice of the ambitious. He avoided all pomp, honours and awards. Dr. Devic is dead. He requested a simple funeral…….. Nonetheless, I do not feel I have betrayed his memory by recounting his life’s work. He loved his science too much not to pardon one of his pupils for writing a brief summary in a medical journal of his life. LYON MEDICAL, 1930.

La sindrome di Devic: un’ipotesi. Giorgi D, Bonomo L. RHEUMATOLOGY 2000; 39: 216-17. 1)“In our opinion, when a neuromyelitis optica (NMO) occurs in association with a systemic disease, such as MS, SLE or APS, it’s not simple to establish whether its occurrence in these patients may be classified as a secondary Devic-like disorder or as a strict Devic’s syndrome.” 2) We would like to know whether the pathophysiology of isolated NMO is different or similar to the pathophysiology of NMO which occurs in the course in course of MS, SLE or APS.

La sindrome di Devic: un’ipotesi 3) On one hand we have patients with NMO who are not suffering from those abnormalities which indicate the presence of a systemic disease. We beleieve that they can be considered as having a “primary Devic’s syndrome”. On the other, we have a group of patients with various clinical/laboratory features, which can probably be classified as suffering from “secondary Devic’s syndrome”; this latter group should include those affected by MS, APS, SLE and those with less than four ARA criteria for SLE diagnosis (Lupus-like diseases).

La sindrome di Devic: un’ipotesi 4) We would like to know whether a “common” pathogenetic factor able to induce the occurrence of NMO in the course of both “primary” and “secondary” Devic’s syndrome exists. 5) Just as some authors during the early 1980s, detected the presence of common serological markers ( the APL) in patients with or without SLE who were suffering from an association of thrombosis, recurrent abortions and thrombocytopenia, we now hope that other authors will demonstrate the existence of one or more common factors which could induce the occurrence of both “primary” and “secondary Devic’s NMO.
