Activation of nuclear factor- B (NK- B, which consists of two subunits, p50 and p65) involves phosphorylation of the inhibitory protein I B by specific kinase(s), with subsequent ubiquitination and proteolytic degradation by the proteasome. The free NF- B then translocates to the nucleus, where it binds to B sites in the promoter regions of inflammatory genes. Activation of the I B- gene results in increased synthesis of I B- to terminate the activation of NF- B.

NF- B may be activated in asthma by a variety of inflammatory signals, resulting in the coordinated expression of multiple inflammatory genes, including cytokines, chemokines, enzymes and adhesion molecules. Additional transcription factors (TF2) interact with NF- B to amplify the expression of particular genes. The cytokines interleukin-1 (IL-1 ) and tumour necrosis factor- (TNF- ) both activate and are regulated by NF- B and may there act as an amplifying feedforward loop. Activation of NF- B, for example by cytokines is blocked by glucocorticoids, both by a direct inhibitory effect of the glucocorticoid receptor binding to the p65 subunit of NF- B, or by stimulating the expression of the inhibitory protein I B- via binding of the glucocorticoid receptor to a positive glucocorticoid response element in the promoter region of the / B- gene. This results in potent inhibition of expression of inflammatory proteins.
